Position Overview:
Work as part of the Behavioral Health Home multidisciplinary team to support the care delivery of persons served in several interdependent areas including self-management support, delivery system design, decision support, clinical information systems, and community linkages.

Title: Case Manager                      

Reports to: Program Supervisor

Department: Behavioral Health Home (BHH)

Classification: Non-Exempt; Union

Essential Functions:

  • Assist persons served to access improved care coordination, linkage, referral, family support and health and wellness promotion.
  • Assist in follow-up care and follow through on recommendations/referrals.
  • Assist with appointment scheduling, and accessing and transporting to services.
  • Identify and address persons served perceived barriers to treatment participation.
  • Skill building/teaching/coaching to help individuals maximize independence in the community.
  • Monitor persons served participation in and response to treatment.
  • Reach out to persons served on a regular basis to assess how they are doing. Brief check-ins, usually fifteen (15) minutes, should be conducted by phone as well as in person.
  • Demonstrate an understanding and appropriate use of Stages of Change, Motivational Interviewing, and skill-based interventions.
  • Understand the philosophy of BHH and have the ability to provide excellent customer skills to our persons served in describing what this enhancement to services can offer.
  • Assist in accessing self-help, peer support groups, wellness centers, and other self-care programs.
  • Teaching and coaching self-advocacy for individuals and families.
  • Attend regular team meetings for developing/sharing medical/behavioral health services which persons served have received, including gaps in care, and their treatment regimens.
  • Assist with entitlements.
  • Meet with Director of Integrated Health on a regular, weekly basis.
  • Attend all required trainings as assigned.
  • Other duties as assigned.

Physical Requirements:
While performing the duties of this job, the employee is regularly required to sit, use hands to finger, handle or feel objects, tools, or controls, and climb stairs. The employee frequently is required to walk, talk and/or hear; Speaking and hearing ability sufficient to communicate effectively by phone or in person at normal volumes; Vision adequate to read correspondence, computer screen, forms, etc. The employee is occasionally required to stand and climb or balance. The employee must occasionally lift and/or move up to 25 lbs. Specific vision abilities required by this position include the ability to adjust focus.

Qualifications:
Associate's degree (or sixty (60) credits equivalent) in a related field OR certification in medical or human services field required. A minimum of two (2) years of behavioral health or human services related experience may be considered in lieu of the degree/certification; bilingual (Spanish/English) required; current Motor Vehicle Drivers' license required; Competent user of technology including e-mail, agency intranet and the electronic client record; Demonstrated ability to work with clients in the field teaching skills necessary for community living and supporting recovery; Excellent written and oral communication skills; CPR/First Aid Certified or eligible.

Hours:
Full Time; Monday - Friday, 8:00 AM - 4:30 PM

Salary:
$19.84/hour, PLUS additional $0.50/hour for individuals who are bilingual (Spanish/English) and pass a proficiency test
