Bride Brook Announces Implementation of PatientPing

SavaSeniorCare Administrative Services LLC is proud to announce implementation of PatientPing for each of its Connecticut Skilled Nursing Facilities to assist each of the Facility’s acute care partners’ efforts to reduce unnecessary hospital readmission while improving quality care and outcomes.
 
PatientPing is a highly effective platform that provides real-time admission and discharge notifications (called Pings) to providers everywhere patients receive care, allowing for immediate engagement and collaboration between providers.
 
PatientPing works to close the gaps in the patient care continuum, leading to improved care, lower costs and safer, more seamless care transitions.
 
With PatientPing, we are now able to more effectively maintain consistency in a patient’s care continuum. For ninety days following discharge from Pendleton Health & Rehab we will receive “Pings” on patients when they are admitted to the emergency room and/or hospital.
 
What This Means For You
On receipt of the notification, Angela Bujnevicie or Britany Whatley, Area Director Care Transition may contact your emergency room or hospital case manager. In the event that the patient does not need an inpatient acute care stay and would be better suited for a skilled nursing facility, we will do our best to ensure readmission to Pendleton Health & Rehab
