Department: Outpatient Clinic

Schedule: Part Time; 2 days/week on first shift (for a total of 16 hours per week)

Eligible for $3,000 - $5,000 Sign-On Bonus!

Salary: LCSW/LPC: $30,000 annually / $36.39 per hour

Position Overview

Job title and Classification:  Clinician (LCSW/LPC) – Exempt; Union

Provide individual and group therapy in a community based outpatient clinic. Using CareLogic software the clinician will document all clinical services provided in accordance with DMHAS and SCSI expectations. Clinician is expected to manage case load assigned including all documentation in a timely manner. Attend team meetings and clinical supervisions.

Essential Functions:

  • Provides individual, group, and family therapy to an assigned caseload. Must meet agency established productivity goals.
  • Completes intake assessments as directed by the Director of Outpatient Services. Completes treatment plan reviews that include updates on client progress at least every ninety (90) days and discharge summaries whenever a case is closed.
  • Conducts crisis assessments as assigned.
  • Produces documentation which is legible and relates to recovery goals as documented in recovery plans.
  • Provides coverage and support for Intake Department as assigned.
  • Documents all client contacts in the client's electronic medical record.
  • Completes all documentation i.e., referrals, treatment plans, reassessment, and progress notes in accordance with the agency's Quality Assurance standards and in a timely manner.
  • Provides appropriate linkage with community resources, e.g., social, residential, and rehabilitation services.
  • Completes client file reviews in the designated timelines as advised and/or written by the Director of Outpatient Services, Senior Director of Quality, or designee.
  • Provides mentorship to students participating in an internship or employees who are in a Counselor or Social Worker role ultimately seeking clinical licensure.
  • Actively participates in or leads supervision, multi-disciplinary team meetings, Risk Review, agency committees and program safety drills.
  • Manages case load, regularly reviews in CareLogic software, performing utilization reviews independently.
  • Performs all other duties as assigned.

Qualifications:

  •  Connecticut Licensed Clinical Social Worker (LCSW) or Licensed Professional Counselor(LPC) required
  • Experience providing assessments, individual and group therapy, discharge planning and referral services to adults with severe mental illness, substance abuse, and co-occurring disorders
  • Must be billable for services provided to clients with Medicare and private insurance coverage
  • CPR/First Aid Certified or eligible
  • Bilingual (Spanish/English) preferred

Physical Requirements:

  • Regularly required to sit, use hands to finger, handle or feel objects, tools, or controls, and climb stairs
  • Regularly required to walk, talk and/or hear
  • Speaking and hearing ability should be sufficient to communicate effectively by phone or in person at normal volumes
  • Vision adequate to read correspondence, computer screen, forms, etc.
  • Specific vision abilities required by this position include the ability to adjust focus
  • Occasionally required to stand and climb or balance
  • Occasionally lift and/or move up to 25 lbs
