Ledge Light Health District Increases Diabetes Management Programs in Eastern CT

The 2016 Community Health Needs Assessment, completed by Ledge Light Health District (LLHD) and Lawrence and Memorial Hospital, reported that diabetes rates have been rising steadily, both in New London County and nationally, mirroring the rising rates of obesity. In response, LLHD has increased the number of diabetes self-management programs it provides throughout SECT.  “Live Well with Diabetes”, developed by Stanford University, is a unique evidence-based, six-week workshop offered at no-cost to adults living with diabetes and/or their caregiver. Live Well with Diabetes has been proven to increase self-management skills and the confidence needed to sustain a healthier lifestyle (increasing physical activity, blood sugar monitoring and the use of stress management techniques), preventing many of the complications associated with the disease. 

Ledge Light Health District, through a grant from the CT Department of Public Health, in collaboration with Senior Resources, will offer three Live Well with Diabetes workshops in 2017.  Each workshop is co-facilitated by certified Live Well with Diabetes educators. The program will introduce tools needed in day-by-day life for persons with diabetes, i.e., monitoring blood sugar, foot care, communicating with family and health care providers, stress management, healthy eating, counting carbohydrates, what to do on sick days, action planning, physical activity and meal planning.

We hope that you will refer your diabetic patients/clients/employees and/or their caregiver or support person to one of our workshops. They have been scheduled in three convenient locations and at different times of the day. To assist you in referring, we have included informational brochures and flyers (please see attached).
