UCFS Receives Level 3 Patient Centered Medical Home Recognition by the National Committee for Quality Assurance (NCQA)

UCFS is pleased to announce that our Plainfield, Griswold and Norwich Health Centers have been recognized as Level 3 (highest level) Patient Centered Medical Home by the National Committee for Quality Assurance (NCQA).  This is the second time UCFS has achieved Level 3 Patient Centered Medical recognition from NCQA for all three primary care locations.

The NCQA Patient-Centered Medical Home standards emphasize the use of systematic, cost-effective, coordinated care that supports quality, access, communication and patient involvement. This recognition is awarded to health care organizations who demonstrate, through a comprehensive review process, that they meet those high standards.
