Who You Are: Sound Community Services, Inc. is seeking team members whose mission is to display compassion, accountability, respect and excellence in our communities. Position Overview: To determine, plan and provide a complete network of aftercare services for individuals to make a successful transition to independent living in the community. Essential Functions: • Develops and reviews recovery plans (according to policy) with person served, identifying their goals and objectives. • Completes crisis plans as needed with person served and other collaborative partners. • Assures appropriate entitlements are in place or completes paperwork to apply for benefits. Develops monthly budget with clients when appropriate. Submits monthly budget by the 25th of each month for those with rep payee status. • Monitors and keeps accurate records or medication compliance for persons who utilize medication monitoring. • Maintains person served record, completing documentation 90% concurrent. • Provides advocacy and crisis intervention as indicated or requested by persons served. • Develops linkage and follow-up program for person served to other support services as needed such as medical evaluation and treatment, social services, employment services and other agencies. • Service Coordinator will see persons served within 24 hours when they are admitted to a higher level of care. Service Coordinator will attend treatment team meetings as scheduled. • Supervises and instructs in Activities of Daily Living as outlined in persons served Recovery Plan. • Attend the mandatory PSRB Acquittee Fundamental Training Program and refresher courses in accordance with DMHAS requirements. Understand the responsibilities of community providers when providing services to an acquittee. • Provides time limited transportation to persons served as well as teaching persons served how to utilize public transportation. • Provides health education to persons served as needed; this may include but it not limited to sex education, communicable disease prevention, etc. • Service Coordinator will conduct ongoing assessments of persons served by completing MGAF forms during quarterly Recovery Plan reviews and the completion of functional assessments (CASIG). • Follows general work rules. • Attends 30 hours clinical in-service annually. • Service Coordinator will complete assessment annually for person served receiving clinical services elsewhere in the community. • Adheres to productivity requirements. Meets the minimum monthly standard. Physical Requirements: While performing the duties of this job, the employee is regularly required to sit, use hands to finger, handle or feel objects, tools, or controls, and climb stairs. The employee frequently is required to walk, talk and/or hear; Speaking and hearing ability sufficient to communicate effectively by phone or in person at normal volumes. Vision adequate to read correspondence, computer screen, forms, etc. The employee is occasionally required to stand and climb or balance. The employee must occasionally lift and/or move up to 25lbs. Specific vision abilities required by this position include the ability to adjust focus. Qualifications: Associate’s degree in a related field required (additional experience may be considered in lieu of a degree); a minimum of two (2) years of behavioral health or human services related experience required (custodial care will not be counted as experience); competent user of technology including e-mail, agency intranet and the electronic client record; excellent written and oral communication skills; CPR/First Aid Certified or eligible; current valid drivers' license is required.
