UCFS Healthcare Recognized by Federal Authorities for Health Care Quality

UCFS Healthcare has received 2023 Community Health Quality Recognition (CHOR) badges for Advancing Health Information Technology and Patient-Centered Medical Home (PCMH) from the U.S. Health Resources Services Administration (HRSA). CHQR badges recognize health center program awardees that have made notable quality improvement achievements in the areas of access, quality, health  equity, health information technology and COVID-19 public health emergency response.

Recognition of the Advancing Health Information Technology (HIT) badge demonstrates that UCFS Healthcare meets all quality criteria for HIT services, including adopting an electronic health record, offering telehealth, engaging patients through health IT and collecting data on social determinants of health.
PCMH recognition is awarded to health centers that embrace the PCMH model of care, which prioritizes a commitment to continuous quality improvement and a patient-centered approach to care.

Jen Granger, President/CEO of UCFS Healthcare stated; “These awards are a testament to our team’s on-going  commitment to our patients. Our staff continue to work to expand access  for our patients beyond the walls of our health centers. This includes Remote Patient Monitoring where we are able care for our patients with chronic conditions from their home.“
